Social Work Supervisor - Case Assistance

Position Description Summary: The Social Work Supervisor- Case Assistance Social Worker (CASW) provides ongoing and short-term case assistance and crisis intervention to the older adults, caregivers, and volunteers who participate in DOROT programs. The CASW collaborates with social workers and other staff throughout the agency to provide deepened support to high need constituents. The CASW oversees DOROT’s Information and Referral Line and supervises the Resource Specialist.

Primary Responsibilities:

Case Assistance
Assess the case assistance needs of seniors, caregivers and volunteers who do not have an assigned social worker or who require extensive support across all DOROT programs. Related programs include: Community Services, Friendly Visiting, Information & Referral, Door to Door, Kosher Meals for the Homebound, and Volunteer Services
  • Conduct biopsychosocial initial assessments, reassessments, brief home visits, hospital/rehabilitation visits, phone calls and in-office meetings to assess clients’ evolving case assistance needs
  • Provide short- and long-term case assistance regarding homecare, benefits and entitlements, medical and mental health, discharge planning, crisis intervention, general and cognitive decline, legal issues, etc.
  • Communicate and collaborate with DOROT social workers and program and other staff involved in senior, caregiver and volunteer cases
  • Refer seniors, caregivers and professionals to DOROT programs and other appropriate community resources based on need and interests
  • Serve as the agency elder abuse and safety point person
  • Oversee EGL medical house call program referrals and consistent tracking and data entry
  • Collect, track and maintain case assessment and related client interactions using DOROT’s online database (CIMS); this involves routine data entry and review of monthly statistics for internal and reports to funders
Information and Referral
  • Oversee operations for the Information and Referral Line: supervise Resource Specialist
  • Collaborate with relevant staff to redesign and maintain updated I&R guide
  • Field inquiries and client concerns from DOROT staff and delegate for appropriate follow up

General Responsibilities:

  • Provide clinical support to and participate in DOROT community-wide intergenerational volunteer programs
  • Special projects as assigned

Required Qualifications:

  • LMSW required; SIFI certified preferred
  • A minimum of 3 years of professional social work experience working with an elderly population
  • At least 2 years of supervisory experience preferred
  • Ability to engage, listen and relate to older adults
  • Knowledge of benefits and entitlements for older adults
  • Exceptional interpersonal, clinical, and assessment skills
  • Strong written and verbal communication skills
  • Strong organizational and presentation skills
  • Ability to take initiative and problem solve
  • Ability to work independently and collaboratively
  • Knowledge of and connections to community and aging resources a plus
  • Experience working with volunteers a plus
  • Proficiency with Microsoft Office Suite 2010. Familiarity with Salesforce a plus.
